การทบทวนวรรณกรรมอย่างเป็นระบบเกี่ยวกับโรคหลายบุคลิกภาพในผู้ใหญ่ ตั้งแต่ปี ค.ศ. 2011-2018

บทคัดย่อ

วัตถุประสงค์ เพื่อศึกษาเกี่ยวกับ Dissociative Identity Disorder (DID) ในแง่ประเด็นศึกษาเกี่ยวกับโรค สาเหตุของการเกิดโรค การประเมินและวินิจฉัย และการบำบัดรักษา วัสดุและวิธีการ งานวิจัยทบทวนวรรณกรรมอย่างเป็นระบบเกี่ยวกับ DID ในผู้ใหญ่ที่ตีพิมพ์ตั้งแต่ ค.ศ. 2011 ถึง 2018 ใน 5 ฐานข้อมูล ได้แก่ Embase, PubMed, ScienceDirect, Scopus และ PsycINFO โดยสืบค้นตามแนวทาง PICo จากนั้นคัดเลือกบทความซำ้ พิจารณาตามเกณฑ์คัดเข้า-คัดออก ประเมินคุณภาพของบทความด้วย JBI’s critical appraisal tools และสกัดข้อมูลด้วยการสังเคราะห์เนื้อหา ผลการศึกษา งานวิจัยที่ผ่านการคัดเลือกมี28 บทความ สกัดข้อมูลพบ 6 ประเด็นส าคัญ ได้แก่
1) ผลกระทบทางลบหลังรับการวินิจฉัยและการใช้ชีวิตประจำวันของผู้ป่วย 2) การประเมินเพื่อวินิจฉัยโรค ทำความเข้าใจ และแยกระหว่างผู้ป่วยจริงกับกลุ่มแกล้งป่วย 3) ตัวแปรที่สัมพันธ์กับโรค DID 4) การเปรียบเทียบระหว่าง DID และโรคจิตเวชอื่น ๆ 5) สาเหตุของการเกิดโรคโดยเฉพาะสัณฐานวิทยาของสมอง และ 6) การรักษาด้วยจิตบำบัด เช่น จิตบำบัดตามแนวทางของ International Society for The Study of Trauma and Dissociation (ISSTD) จิตบำบัดแบบเปลี่ยนโครงสร้างพลวัตร (dynamic deconstructive psychotherapy: DDP) และอาชีวบำบัดโดยการวิ่ง สรุป การศึกษาครั้งนี้ทำให้ทราบประเด็นการศึกษาเกี่ยวกับ DID ที่ทันสมัยมากขึ้น ในเชิงสาเหตุการป่วย
มีการศึกษาเกี่ยวกับสมองซึ่งมีการอธิบายสาเหตุการป่วยที่เป็นวิทยาศาสตร์มากขึ้นและเป็นแนวทางในการศึกษาต่อในอนาคต การประเมินและวินิจฉัยนิยมใช้ SCID-D ส าหรับการบำบัดรักษามีเป้าหมายเพื่อลดภาวะ dissociation ซึ่งมีส่วนช่วยให้ผู้ป่วยใช้ชีวิตจำวันได้ดียิ่งขึ้น
