บทคัดย่อ
วัตถุประสงค์ (1) เพื่อประเมินประสิทธิผลของคู่มือสนทนาการวางแผนดูแลล่วงหน้า (ACP-CG) ต่อการรับรู้สมรรถนะแห่งตนในการทำการวางแผนดูแลล่วงหน้า (ACP) (2) เพื่อประเมินประสิทธิผลของคู่มือ ACP-CG บทเรียนอิเล็กทรอนิกส์เรื่องการสนทนาเพื่อวางแผนดูแลล่วงหน้า (ACP e-Learning) ต่อการรับรู้สมรรถนะแห่งตนในการทำ ACP (3) เพื่อเปรียบเทียบการรับรู้สมรรถนะแห่งตนในการทำ ACP ระหว่างกลุ่มที่ใช้คู่มือ ACP-CG กับกลุ่มที่ใช้คู่มือ ACP-CG ร่วมกับ ACP e-Learning (4) เพื่อวิเคราะห์อุปสรรคและข้อเสนอแนะสำหรับการวางแผนและพัฒนางานวิจัยการวางแผนดูแลล่วงหน้า
วัสดุและวิธีการ เป็นการวิจัยกึ่งทดลองแบบมีกลุ่มเปรียบเทียบที่มีการวัดผลก่อนหลัง กลุ่มตัวอย่างเป็นแพทย์ประจำบ้านอายุรศาสตร์ 13 คน โดยเลือกตัวอย่างแบบเจาะจง แบ่งเป็นกลุ่มทดลอง-1 ที่ได้รับคู่มือ 8 คน กลุ่มทดลอง-2 ที่ได้รับคู่มือร่วมกับ e-Learning 5 คน หลังการทดลองมีผู้ให้ข้อมูลเชิงคุณภาพ 15 คน เลือกตัวอย่างแบบเจาะจง เครื่องมือที่ใช้ ได้แก่ (1) คู่มือ ACP-CG (2) ACP e-Learning (3) แบบวัดการรับรู้สมรรถนะแห่งตนเกี่ยวกับการวางแผนดูแลล่วงหน้าฉบับภาษาไทย (ACP-SE-Thai) (Cronbach's α = 0.928) และ (4) แบบบันทึกการสัมภาษณ์ วิเคราะห์ข้อมูลโดยใช้ Wilcoxon signed-rank test และ Mann-Whitney U test และการวิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เนื้อหา
ผล (1) คู่มือ ACP-CG ช่วยให้การรับรู้สมรรถนะแห่งตนในการทำ ACP ในกลุ่มทดลอง-1 สูงขึ้น (p < 0.05) (2) คู่มือ ACPCG ร่วมกับ ACP e-Learning ช่วยให้การรับรู้สมรรถนะแห่งตนในการทำ ACP ในกลุ่มทดลอง-1 สูงขึ้น (p < 0.05) (3) กลุ่มทดลอง-1 และกลุ่มทดลอง-2 มีการรับรู้สมรรถนะแห่งตนในการทำ ACP ทั้งก่อนการทดลองและติดตามผล 6 เดือนไม่แตกต่างกัน (4) การวางแผนและพัฒนางานวิจัยในการทำ ACP ได้แก่ การปรับปรุงเครื่องมือวิจัย การติดตามผู้เข้าร่วมวิจัย และการจัดกิจกรรมอภิปรายกลุ่มเรื่องการทำ ACP
สรุป การศึกษานำร่องนี้มีข้อจำกัดในการขยายผลการศึกษาและการยืนยันประสิทธิผลของคู่มือ ACP-CG และ ACP e-Learning แต่ก็ทำให้ได้เครื่องมือต้นแบบที่ช่วยเพิ่มการรับรู้สมรรถนะแห่งตนในการทำ ACP ให้กับแพทย์ประจำบ้านที่ยังไม่มั่นใจในการทำ ACP ได้ และยังได้แบบวัด ACP-SE ฉบับภาษาไทย (ACP-SE-Thai) ที่มีค่าความเชื่อมั่นสูงมากอีกด้วย
