A Development of Diabetes Mellitus Care Model in Health Promotion Hospital, Roi-Et Province.

Abstract

          The purposes of this Action Research were 1) to evaluate the situation of Diabetes Mellitus care in Health Promotion Hospital under Roi-Et provincial health office, 2) to design a development of Diabetes Mellitus Care model and 3) to experiment and evaluate a model. The research methodology was divided into 3 phases. At the first phase, collected the questionnaires by 6 causal factors of Diabetes Mellitus Care in 148 sample sizes that taken by proportional cluster random sampling techniques and analyzed data by descriptive statistics. The second phase, designed and developed Diabetes Mellitus Care model based on the results of the first phase and applied Chronic Care Model and PDCA cycle. A development of Diabetes Mellitus Care model was created by workshops, focus groups and brain storming in the research participants. And the third phase, experimented and evaluated a development of Diabetes Mellitus Care model in a Health Promotion Hospital for 4 months and compared by Percentage Difference.

          Results were found that: the overall factors of Diabetes Mellitus Care in Health Promotion Hospital under Roi-Et provincial health office have practiced in a high level (= 3.99 , SD = .475) and then has taken to develop Diabetes Mellitus Care model based on Chronic Care Model and PDCA cycle applying by the consists of 6 steps 1) Self – Assessment 2) Analysis 3) Design 4) Implementation 5) Evaluation and Monitoring and 6) Feedback. A development Diabetes Mellitus Care model in a Health Promotion Hospital target showed higher Percentage Difference as a whole (57.96%).
