การรอดชีพของผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่รักษาโดยการฟอกไตเปรียบเทียบกับการดูแลแบบประคับประคองในโรงพยาบาลชุมชน

บทคัดย่อ
ที่มา: ข้อมูลการรอดชีพของผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายในประเทศไทยยังมีจำกัด งานวิจัยนี้มีวัตถุประสงค์เพื่อเปรียบเทียบการรอดชีพของผู้ป่วยที่ได้รับการฟอกไตกับการดูแลแบบประคับประคองในโรงพยาบาลชุมชน
แบบวิจัย: การศึกษาเชิงสังเกตแบบย้อนหลัง
วัสดุและวิธีการ: เก็บข้อมูลผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายอายุ ≥ 18 ปี ระหว่างเดือนมกราคม พ.ศ. 2560 ถึงเดือนธันวาคม พ.ศ. 2566 และติดตามจนถึงเดือนธันวาคม พ.ศ. 2567 บันทึกข้อมูลโดยใช้แบบบันทึกข้อมูล วิเคราะห์การรอดชีพแบบ intention-to-treat โดยใช้ Kaplan–Meier และ Laplace regression ปรับด้วยตัวแปรอายุ Charlson Comorbidity Index อัตราการกรองของไต และการได้รับยา erythropoiesis-stimulating agents
ผลการศึกษา: ผู้ป่วยทั้งหมด 192 ราย อายุ 18-90 ปี (เฉลี่ย ± ส่วนเบี่ยงเบนมาตรฐาน 61.9 ± 15.0 ปี) ค่ามัธยฐานการรอดชีพของกลุ่มฟอกไตนานกว่ากลุ่มประคับประคอง (5.4 ปี, 95% confidence interval (95%CI:) 4.2–6.3 เทียบกับ 1.9 ปี, 95%CI: 1.4–2.6, p < 0.001) อย่างไรก็ตาม เมื่อปรับปัจจัยรบกวนแล้ว ความแตกต่างลดลงและไม่มีนัยสำคัญทางสถิติ (1.1 ปี, 95%CI: -1.7–3.8, p = 0.440) การวิเคราะห์กลุ่มย่อยพบว่า ผู้ที่อายุน้อยกว่า 65 ปี มีแนวโน้มได้ประโยชน์จากการฟอกไตมากกว่า (2.7 ปี, 95%CI: 0.12–5.3, p = 0.04)
สรุป: การฟอกไตมีแนวโน้มเพิ่มระยะเวลาการรอดชีพ โดยเฉพาะในผู้ป่วยอายุน้อยกว่า 65 ปี การเลือกแนวทางการรักษาจึงควรอยู่บนพื้นฐานของการสื่อสารและการตัดสินใจร่วมกันระหว่างแพทย์ ผู้ป่วย และครอบครัว
