Abstract
ในยุคปัจจุบัน ระบบสุขภาพกำลังเปลี่ยนแปลงจากการเน้นการดูแลในสถานพยาบาลเพียงอย่างเดียว ไปสู่การดูแลสุขภาพแบบองค์รวม (holistic care) ที่ต่อเนื่องในทุกมิติของชีวิตผู้ป่วย แนวคิดการดูแลต่อเนื่องแบบบูรณาการนอกโรงพยาบาล (Integrative Continuing Care Beyond Hospital) จึงกลายเป็นแนวทางสำคัญในการพัฒนาระบบสุขภาพที่มีคุณภาพและยั่งยืน โดยมีแพทย์เวชศาสตร์ครอบครัวเป็นแกนกลางในการประสานงานและเชื่อมโยงการดูแลระหว่างโรงพยาบาล ชุมชน และครอบครัวอย่างไร้รอยต่อ
