Comparing knowledge, attitude, and practice toward advanced care planning among palliative and renal healthcare professionals after advanced care plan training program: 12 health sectors in Thailand knowledge, attitude, and practice toward advanced care planning among healthcare professionals

Abstract
Background & Objectives: Advance care planning(ACP) is a process that supports patients and families in understanding and sharing their goals and preferences regarding future medical care. The knowledge, attitude, and training are essential for conduct and adherence to ACP. The modified Serious Illness Conversation(mSICG) ACP workshop was planned to train health providers. The authors aimed to evaluate and compare knowledge, attitude, and practice(KAP) toward ACP in palliative(PC) and chronic kidney disease(CKD) providers before and after the training workshop.
Design: pre-posttest quasi-experimental design
Material & Methods: The study was conducted all over 12 health sectors across Thailand. The pretest and posttest KAP were completed before and after the mSICG ACP workshop immediately. The workshop included an ACP lecture, VDO feedback, and role-play practice.
Results: Of the 483 participants, 453 were female, and 49% were aged 46-60 years old. Half participants were PC nurses, 30% were CKD nurses, 10.8% were PC doctors and 2.5% were CKD doctors. The knowledge and attitude of each discipline were nearby each other, PC team had more practice scores than CKD team. After the mSICG training, knowledge of CKD nurses, and attitude of PC and CKD nurses were improved significantly, P<0.001. PC doctor's knowledge and attitude were higher without significant level.
Conclusion: The KAP of the Thai PC team is higher than CKD team. The knowledge and attitude were improvable by training. The mSICG-ACP workshop is one of the effective ACP training programs and should be implemented for CKD and other advanced disease care providers.
