Abstract

Background: Type 2 diabetes mellitus (T2DM) is a major global and national health problem with increasing prevalence. Intensive behavioral intervention has been proposed as a promising approach to improve glycemic control and reduce diabetes-related complications.

Objectives: To evaluate the effectiveness of intensive behavioral intervention in patients with T2DM

Design: Retrospective cohort study

Methods: Medical records of diabetic patients attending a Primary Care Clinic from October 2023 to October 2025 were reviewed. Participants were classified into an intensive behavioral intervention group and a standard care group. The primary outcome was the change in HbA1c levels before and after intervention.

Results: A total of 480 patients were included, comprising 96 patients in the intensive lifestyle modification group and 384 patients in the control group. The intensive group had fewer older adults (≥ 60 years) than the control group (51.0% vs. 59.1%, p = 0.024), with a predominance of females in both groups (69.8% vs. 68.0%). The intensive lifestyle modification group demonstrated a significantly greater reduction in HbA1c levels than the control group (−6.6%; 95%CI −10.8 to −2.5; p = 0.001).

Conclusions: Intensive behavioral intervention in primary care settings is effective in improving glycemic control among patients with T2DM and can be applied to enhance the quality of diabetes care in community- based primary healthcare systems.
