Prevalence and Factors Associated with Caregiver Burden Among Caregivers of Bedridden Elderly in Mueang Surin District, Surin Province, Thailand

ABSTRACT
Background: Thailand has become an aging society, with the number of bedridden elderly steadily increasing in the Surin Province. Family members, who serve as primary caregivers, frequently experience caregiver burden. However, empirical evidence on this issue in Mueang Surin District remains limited. This study aimed to determine the prevalence and factors associated with caregiver burden among caregivers of bedridden elderly individuals to provide essential baseline information for developing community-based caregiver support programs.
Design: A cross-sectional study
Methods: A cross-sectional study was conducted among caregivers of bedridden elderly individuals in Mueang Surin District, Surin Province, Thailand, between January and May 2025. Data were collected using a structured questionnaire comprising general information, the Zarit Burden Interview-Thai version (ZBI-12), and the Patient Health Questionnaire-9 (PHQ-9). The ZBI-12 total score ranges from 0-48, with scores ≥ 20 indicating high burden, while the PHQ-9 score ranges from 0-27, with scores ≥ 9 indicating depression. Data were analyzed using descriptive statistics and multivariable logistic regression that controlled for confounding factors such as age, gender, socioeconomic factors of caregivers, and causes of bedridden elderly that had clinical significance.
Results: A total of 150 caregiver-bedridden elderly pairs were included. The prevalence of caregiver burden was 16%, with a mean ZBI score of 11.45 ± 8.20. Factors significantly associated with caregiver burden were caregiver depression (OR = 1.31, 95%CI: 1.15-1.49, p < 0.001) and caring for a bedridden elderly person due to other causes (OR = 9.09, 95%CI: 1.64-50.48, p = 0.012). No significant associations were found between caregiver burden and socioeconomic or elderly characteristics.
Conclusion: Caregiver burden was significantly associated with caregiver depression and bedridden status due to other causes.
