Predictive Factors of Malnutrition and Risk of Malnutrition Among the Elderly in Mueang Phitsanulok Primary Care Unit
Main Article Content
Abstract
Background: Malnutrition is a common health problem in the elderly. Malnutrition screening is typically done using the Mini Nutritional Assessment (MNA). The MNA is complex, has many questions, and requires a long time for assessment. It is not suitable for screening malnutrition in large populations in primary care units. This study aimed to identify predictive factors and develop a screening tool for malnutrition and risks in the elderly.
Methods: A cross-sectional analytical study was conducted among residents aged 60 and older in the Mueang Phitsanulok Primary Care Unit area, with 385 participants. Data were collected through a questionnaire from June to August 2024. The data were analyzed to identify predictive factors using multivariable risk ratio regression. A screening tool was developed based on the risk ratio regression coefficients.
Results: The top three predictive factors of malnutrition and risk of malnutrition are loss of appetite in a period of three months (risk ratio (RR) 2.45, 95% confidence interval (CI) 1.96-3.05), history of decreased body weight (RR 1.86, 95% CI 1.57-2.20), and body mass index < 23 kg/m2 (RR 1.53, 95% CI 1.31-1.78). The new screening tool has an area under the receiver operating characteristic (ROC) curve of 96.72%, with a sensitivity of 87.6% and specificity of 92.7%.
Conclusion: The top three predictive factors of malnutrition and risk of malnutrition are loss of appetite within three months, history of decreasing body weight, and body mass index < 23 kg/m2. Developing an effective screening tool for malnutrition and risk of malnutrition that reduces the number of questions and choices, shortens screening duration, and can be used for large populations in primary care units
