บทคัดย่อ
การวิจัยเรื่องนี้มีวัตถุประสงค์เพื่อ (1) วิเคราะห์ภาพรวมสถานการณ์ของกลไกการบริหารจัดการระบบบริการสุขภาพเขตพื้นที่กรุงเทพมหานคร (2) วิเคราะห์ปัญหาของกลไกการบริหารจัดการระบบบริการสุขภาพเขตพื้นที่กรุงเทพมหานคร และ(3) ศึกษาแนวทางในการพัฒนากลไกการบริหารจัดการระบบบริการสุขภาพเขตพื้นที่กรุงเทพมหานคร เป็นการวิจัยเชิงคุณภาพ ผู้ให้ข้อมูลสำคัญ ได้แก่ กลุ่มผู้บริหารของกรุงเทพมหานครด้านสาธารณสุข 9 คน กลุ่มบุคลากรทางการแพทย์และอาสาสมัครสาธารณสุขกรุงเทพมหานคร 9 คน และกลุ่มนักวิชาการ/ผู้เชี่ยวชาญ 2 คน รวม 20 คน คัดเลือกแบบเจาะจง จาก
ผู้มีประสบการณ์และความรู้เฉพาะด้าน เครื่องมือวิจัย คือแบบสัมภาษณ์ วิเคราะห์ข้อมูลโดยใช้การสรุปความแบบพรรณนา
ผลการวิจัยพบว่า (1) ระบบบริการสุขภาพในเขตกรุงเทพมหานคร แม้มีการแบ่งโซนสุขภาพและจัดตั้งโรงพยาบาลใหม่ แต่ยังขาดหน่วยบริการระดับทุติยภูมิ ประชาชนยังเข้าถึงบริการปฐมภูมิได้ยาก ปัญหากำลังคนยังคงขาดแคลน โดยเฉพาะแพทย์ พยาบาล และเภสัชกร ด้านระบบข้อมูลมีการพัฒนาแต่ยังขาดการเชื่อมโยงและความน่าเชื่อถือ ด้านยาและเทคโนโลยีมีนวัตกรรมใหม่แต่ยังบริหารจัดการไม่เป็นระบบ งบประมาณไม่เพียงพอ การอภิบาลระบบมุ่งเน้นกระจายบริการสู่ชุมชนและการมีส่วนร่วมของประชาชน แต่ยังขาดการประสานงานที่เป็นเอกภาพ (2) ปัญหาหลัก ได้แก่ ความเหลื่อมล้ำในการเข้าถึงบริการ เครือข่ายบริการไม่ครบ ส่งผลกระทบต่อระบบส่งต่อ กำลังคนไหลออกจากระบบ ข้อมูลสุขภาพไม่เชื่อมโยง ขาดเครื่องมือและแผนการใช้ยาอย่างเป็นระบบ งบประมาณไม่สอดคล้องกับความต้องการ และหน่วยงานยังขาดความร่วมมือ (3) แนวทางพัฒนา ควรยกระดับบริการสุขภาพให้ครอบคลุมครบวงจร สร้างโรงพยาบาลประจำเขต เพิ่มบุคลากรและปรับค่าตอบแทน ใช้เทคโนโลยีลดภาระงาน สร้างฐานข้อมูลกลางเชื่อมโยงทุกหน่วยงาน ปรับระบบบริหารยาให้อัตโนมัติ จัดสรรงบประมาณตามพื้นที่ และส่งเสริมความร่วมมือภายใต้กลไกธรรมาภิบาล
