บทคัดย่อ
เวชศาสตร์ครอบครัวและการดูแลสุขภาพปฐมภูมิจะพัฒนาไปในทิศทางใด ต้องเข้าใจ ที่มาความจำเป็น ของสองส่วนนี้อย่างลึกซึ้ง การศึกษาประวัติศาสตร์เป็นส่วนหนึ่งของการเรียนรู้ที่สำคัญ บทความนี้เป็นการทบทวนวรรณกรรม เพื่อศึกษาที่มาของพัฒนาการเวชศาสตร์ครอบครัวและการดูแลสุขภาพปฐมภูมิซึ่งเกิดขึ้นควบคู่กันก่อนคำประกาศอัลมา อตา ปี พ.ศ.2521 ที่เรียกร้องให้พัฒนาการดูแลสุขภาพปฐมภูมิ ให้ความสำคัญกับการดูแลที่ครอบคลุมลดปัญหาความเหลื่อมล้ำ ติดตามการเปลี่ยนแปลงที่เกิดขึ้น 40 ปีต่อมา ทบทวนวิเคราะห์จุดอ่อนจุดแข็งของการพัฒนา ที่ต่างกันไปในแต่ละบริบท และการออกคำประกาศแอสทานาในปี พ.ศ. 2561
การพัฒนาการแพทย์บนฐานวิทยาศาสตร์ ทำให้การแพทย์มีความก้าวหน้าลงลึกพัฒนาเป็นสาขาเฉพาะทาง วิชาเวชศาสตร์ครอบครัวเกิดขึ้นจากข้อค้นพบของการทำงานในบริบทปฐมภูมิที่มีการเรียนรู้ร่วมกันระหว่างทีมสุขภาพเพื่อให้การดูแลสุขภาพที่ครอบคลุม บูรณาการเป็นองค์รวม ที่ต้องอาศัยเนื้อหาความรู้และทักษะต่างไปจากบริบทของโรงพยาบาล หลังคำประกาศอัลมา อตา ประเทศไทยได้มีการพัฒนาการสาธารณสุขและระบบสุขภาพชุมชนมาอย่างเข้มแข็ง ทั้งนี้การพัฒนาต่อไปในอนาคตจำเป็นต้อง สนับสนุนการศึกษาวิจัยและการเรียนรู้ร่วมกันของทีมสุขภาพในการพัฒนาการจัดบริการปฐมภูมิและสร้างการมีส่วนร่วม
ของทุกภาคส่วนเพื่อพัฒนาศักยภาพและสร้างคุณค่าของระบบสุขภาพปฐมภูมิอย่างยั่งยืน
