บทคัดย่อ
ที่มาและวัตถุประสงค์: งานวิจัยปฐมภูมิจะเป็นหลักฐานเชิงประจักษ์ในการรักษาผู้ป่วยให้สอดคล้องกับความหลากหลายของปัจจัยทางสังคมที่มีผลต่อสุขภาพได้ดียิ่งขึ้น บทความนี้เพื่ออธิบายความหมายของงานวิจัยปฐมภูมิในบริบทที่แตกต่างกัน เน้นย้ำความจำเป็นของงานวิจัยปฐมภูมิในงานเวชศาสตร์ครอบครัว และถอดบทเรียนความสำเร็จการพัฒนางานวิจัยปฐมภูมิจากต่างประเทศ
เนื้อหา: งานวิจัยปฐมภูมิ หมายถึง งานวิจัยที่ทจำในบริบทปฐมภูมิหรืองานวิจัยที่มีประเด็นเกี่ยวข้องกับบริการปฐมภูมิ แบ่งได้ 5 มิติ ได้แก่ ความรู้พื้นฐาน (basic knowledge) การแก้ปัญหาทางคลินิก (clinical problem solving) บริการสุขภาพ (health services andpractice implementation) ระบบบริการ (health systems and policy context) และการศึกษา (Education) เวชศาสตร์ครอบครัวมีลักษณะสำคัญ 3 ประการที่บ่งชี้ความจำเป็นต้องมีงานวิจัยเฉพาะสาขา ได้แก่ การมีเอกลักษณ์ของระบบนิเวศน์ (unique ecology)บริบทของการรักษา (context of care) และความรับผิดชอบต่อชุมชน (accountability to community) ปัจจัยความสำเร็จในการสร้างงานวิจัยปฐมภูมิ ได้แก่ การสร้างหลักสูตรฝึกอบรมวิจัยปฐมภูมิ การประสานให้สถาบันการศึกษาและองค์กรที่เกี่ยวข้องพัฒนาหน่วยสนับสนุนวิจัย การสนับสนุนแพทย์ในระบบบริการให้มีส่วนร่วม การหาความร่วมมือระหว่างประเทศ
สรุป: บริการปฐมภูมิและเวชศาสตร์ครอบครัวต้องการงานวิจัยที่มีความจำเพาะต่อบริบทของปฐมภูมิ การสนับสนุนให้เกิดการสร้างงานวิจัยจะเป็นองค์ความรู้ในการพัฒนางานบริการปฐมภูมิให้ประชาชนมีสุขภาพที่ดีอย่างยั่งยืน
