Integrated Community-Based Long Term Care Model Using Community Participation for Dependent Elders at Dongbang Promoting Hospital in Mueang District, Ubon Ratchathani Province

Abstract
The research aimed to 1) analyze the context and long-term care situation of elderly people by the community network, and 2) develop the integrated community-based long term care model using community participation for dependent elders. The samples were 316 elderly people. The target group for the development of the model was composed of 35 persons who had experiences or were directly related to the care for the elderly. They were selected via snowball sampling. The instruments were in-depth interviews, questionnaire and focus group discussion. The statistics were percentage, mean, standard deviation. Content analysis was also utilized. The results revealed that family members were the main caregivers to provide the elderly with daily care while the community network and government agencies provided care for the elderly by visiting home and offered them health care at home. The integrated community-based long term care model using community participation for dependent elders consisted of 1) development of potential of the elderly/caregivers, 2) continuous care for them in the community, 3) proactive services, 4) medical services, 5) building network and participation, and 6) information reception/consultation.
