บทคัดย่อ

ทักษะการสื่อสารด้วยความเห็นอกเห็นใจ (Empathic Communication Skills) หรือ EC คือพื้นฐานความสัมพันธ์อันดีของแพทย์และคนไข้ ปัจจุบันมีการใช้ละครด้นสดประยุกต์มาบูรณาการกับการฝึกทักษะ EC ในนักศึกษาแพทย์ แต่ยังขาดวิจัยที่ใช้ทฤษฎีการแสดงมาประยุกต์ในเชิงปฏิบัติและอธิบายถึงลักษณะของหลักสูตรโดยละเอียด แนวคิดของคอนสแตนติน สตานิสลาฟสกี (Konstantin Stanislavski) นักการละครผู้คิดค้นระบบฝึกฝนนักแสดงเพื่อให้สวมบทบาทได้สมจริง มีความสัมพันธ์กับทักษะ EC และมีศักยภาพในการใช้กับนักศึกษาแพทย์ได้ บทความนี้อธิบายการออกแบบหลักสูตรการแสดงเพื่อความเห็นอกเห็นใจ ซึ่งบูรณาการทักษะ EC กับแนวคิดของสตานิสลาฟกี 3 ด้านได้แก่ ภาวะสร้างสรรค์ (Creativity state of mind) การเชื่อมโยง (Communion) และจินตนาการ (Creative imagination) หรือ 3C โดยทดลองใช้กับนักศึกษาแพทย์กลุ่มนำร่องและกลุ่มประเมินผลรวม 27 ราย เก็บข้อมูลเชิงปริมาณผ่านการทำแบบประเมินความพึงพอใจหลังเข้าร่วม เก็บข้อมูลเชิงคุณภาพผ่านการสัมภาษณ์เชิงลึกก่อนและหลังเข้าร่วมหลักสูตร ตัวอย่าง 19 รายได้ทดลองสื่อสารกับผู้ป่วยจำลองก่อนและหลังเรียนเพื่อประเมินทักษะ EC ผลวิจัยพบว่าหลักสูตรมีผลลัพธ์ในเชิงบวกทั้งด้านความพึงพอใจและทักษะการสื่อสารของผู้เรียนที่เปลี่ยนแปลงไป หลักสูตรมีความยาว 3 วัน วันละ 6 ชั่วโมง เป็นเส้นทางเรียนรู้ที่ช่วยให้ผู้เรียนเชื่อมโยงกับตนเอง ผู้อื่น และคนไข้ได้ตามลำดับ โดยใช้ภาวะสร้างสรรค์ช่วยให้ผู้เรียนมองเห็นและฟังผู้อื่นได้ดีขึ้น ใช้การแสดงพัฒนาจินตนาการและการเข้าใจอารมณ์ผู้อื่น โดยผสมการฝึกฟังเชิงลึกอยู่ตลอดทั้งหลักสูตร กระบวนกรใช้หลักการสอนแบบวิโอลา สโปลิน (Viola Spolin) ช่วยให้ผู้เรียนรู้สึกปลอดภัยและพร้อมแสดง หลักสูตรที่ประยุกต์จากทักษะ 3C มีความเหมาะสมกับนักศึกษาแพทย์ของไทย มีแนวทางประยุกต์ใช้ได้หลากหลาย เช่น ใช้เป็นวิชาเลือกเรียนอิสระ ใช้เป็นกิจกรรมพิเศษในระยะสั้น หรือประยุกต์ใช้ในวิชาด้านการสื่อสารของแพทย์
